The registration desk at Providence St. Mary Medical Center is in the main lobby, just inside the front entrance facing Fifth Ave. This will be your first stop, whether you are coming in for lab work, imaging tests such as x-rays and mammograms, hospital services, except for emergency procedures and doctor’s appointments. After your registration information is collected, you will be directed to the department that is providing your services. If you have any difficulty finding your way around, please just ask a staff member to assist you. We are happy to provide directions or an escort.
For emergencies, please go directly to the Emergency Department at the Fifth Ave. entrance.
For your appointment with a physician, please check in directly at that physician’s office unless you are directed to do otherwise. If you are uncertain where your physician is located, call 509-526-3333, and we will provide you with that information.
If you’re coming to Providence St. Mary Medical Center for a scheduled procedure, your doctor’s staff will likely give you specific instructions on what to bring.
General items you should bring for an overnight stay:
- Photo ID and insurance card
- Co-payment (if required by your insurance plan)
- Full list of medications with dosages (including over-the-counter supplements)
- List of allergies (foods, medications, etc.)
- Copy of your advance directives
- Pajamas, robe, slippers
- Toiletries
Please leave medications and valuables at home.
All medications you take while a patient at Providence St. Mary Medical Center should be prescribed by your attending physician, dispensed by the hospital pharmacy and administered by a nurse. As a patient, you aren’t allowed to administer your own medications – including over-the-counter and herbal supplements – and you shouldn’t keep medication in your room unless ordered by a doctor.
Why are we so strict about this? It’s for your protection – and so we can deliver the best medical outcome possible. We need to know exactly which medications you’re taking so we can gauge how your body responds to them.
Please don’t bring valuables – such as jewelry – to the hospital. If you accidentally bring valuables to the hospital, please send them home with a friend or family member. Or, ask a staff member to put them in our safe. Providence St. Mary Medical Center is not responsible for lost items.
If you lose something, please notify your nurse immediately. We’ll make every effort to help you find it. Unclaimed items are turned over to the security office. To ask about lost items, call extension 2165.
- Advance Directives: Advance directives are legal documents used to communicate your preferences about future health care.
- Imaging Records Release: Use this form if you’d like your diagnostic imaging data and reports released to another medical facility.
- Imaging Records Pick Up: Use this form to authorize someone else to pick up your diagnostic imaging data and reports.
- Medical Records Release: Use this form to request a copy of your medical records.
A Medication Assistance Program (sometimes referred to as a Prescription Assistance Program) is a program offered by medication makers.
These programs help people who cannot afford their medications or medical supplies get them for free or at a low cost, and often have different eligibility criteria and ways to apply.
