Nutrition and Diabetes Wellness

Whether your quality of life is affected by diabetes, heart disease, high cholesterol or other chronic conditions, our Nutrition and Diabetes Wellness program educates and supports patients in managing their condition through individualized nutrition planning and lifestyle modifications.

Our Nutrition and Diabetes Wellness team features registered dietitians and nationally certified diabetes educators. Together, we work with you to develop an individualized care plan that fits your lifestyle and can help you successfully reach your wellness goals.
The Nutrition and Diabetes Wellness Program at Providence Queen of the Valley Medical Center is recognized by the Association of Certified Diabetes Care Education Specialists as a quality diabetes self-management education program.
Diabetes education will help you learn how to manage your diabetes and be as healthy as possible by focusing on seven self-care behaviors including: healthy eating, being active, monitoring, taking medication, problem solving, healthy coping and reducing risks.
In addition to group classes, physicians can refer patients for one-on-one education on specific topics such as insulin training and continuous glucose monitoring.
As an affiliate of the Sweet Success California Diabetes and Pregnancy Program (CDAPP), our program offers comprehensive preconception, prenatal and post-partum education to pregnant women with diabetes or who develop diabetes during pregnancy.
Group and/or individual instruction on meal planning, glucose meter use and diabetes care are provided. Each patient will be individually monitored throughout pregnancy by a Certified Diabetes Care and Education Specialist.
Medical Nutrition Therapy can play a life changing role in reducing complications with some chronic diseases, including diabetes, kidney disease, high blood pressure and more.
Through individualized appointments with registered dietitians, patients are empowered with the knowledge, skills and resources needed to manage their health condition effectively, optimize their nutritional choices and improve their overall quality of life.
Please contact your primary care physician for a referral. Once the nutrition team receives your referral, they will contact you to determine your health goals and schedule you for a group class or nutrition appointment.

The Nutrition and Diabetes Wellness programs are covered by most insurance and Medicare Part B, based on diagnosis. Please check with your insurance company to confirm your coverage.
