HIPAA Notice
Protecting the privacy of our patients' medical information is one of Covenant Health's top priorities. All staff members, from physicians to volunteers, are required to comply with federal Health Insurance Portability and Accessibility Act (HIPAA) regulations regarding the access to patients' personal information and medical records.
Access to Medical Records
Patients are welcome to access and request copies of their medical records and request corrections if they identify errors and mistakes. Covenant will respond to any request for information within 30 days or less.
Notice of Privacy Practices
Covenant will provide patients with a copy of our privacy policy on their first visit. This policy outlines how we may use personal medical information and patients' rights to control that information under HIPAA. Should the privacy policy change significantly, we will mail you a revised version. Patients are also entitled to ask Covenant to restrict the use or disclosure of their personal health information beyond the practices disclosed in our policy. Limits on Use of Personal Medical Information
To ensure our patients receive the best quality care, doctors, nurses and other providers will sometimes need to share necessary treatment information. This may include sending information about you to a specialist as part of a referral, sending billing information to your insurance company or Medicare, or compiling your data with other patients' to compare the effectiveness of various treatment methods.
Confidential Communications
Patients are welcome to ask physicians and staff to take additional steps to protect their privacy, such as contacting them at a home number rather than a work number.
Complaints
If you have questions or would like additional information, or if you believe your privacy rights have been violated, you can contact the Covenant Health Privacy Officer via mail at 3615 19th Street, Lubbock, Texas 79410 or call 888-294-8455. You may also file a complaint with the U.S. Department of Health and Human Services Office of Civil Rights, 1301 Young Street, Suite 1169, Dallas, Texas 75202. Filing a complaint will not negatively affect the treatment or coverage that you receive.
