Frequently Asked Questions
Suppliers visiting Providence facilities must adhere to the following general health and masking requirements:
- Follow masking requirements of the setting
- Refer to posted signage upon entry for specific guidelines
- For questions, consult the caregiver you are meeting with or a local leader
- Tuberculosis Testing - Tuberculosis testing; IGRA or Q-Gold blood test or two-step tuberculin skin test current within the last 12 months, and annual as per ministry requirements. If history of positive please provide copies of chest x-ray results after positive TB test and medical clearance note from your provider.
- Measles, Mumps, Rubella (MMR) - Documentation of 2 MMR’s at least four weeks apart after the age of one and/or positive laboratory titer or signed declination (Rubella is required in Alaska in acute facilities, skilled nursing facilities, and ambulatory surgery centers)
- Varicella (Chicken pox) - Documentation of 2 doses of varicella at least four weeks apart and/or positive laboratory titer or signed declination
- Hepatitis B (Hep B) - Documentation of Hepatitis B vaccinations (series of 3 Engerix or Recombivax or 2 Heplisav) and positive laboratory titer or signed declination where required. (Hep B vaccination is required in Alaska in acute facilities, skilled nursing facilities, and ambulatory surgery centers)
- Tetanus, Diphtheria & Pertussis (Tdap) - Documentation of vaccination/booster or signed declination
- Annual influenza vaccine - Documentation of vaccination or signed declination, including reason for declining. Must follow masking requirements of setting.
- COVID vaccination - Documentation of updated (most current) COVID-19 vaccine or a written declination for medical or religious purposes preferred. Please refer to local policy for masking requirements. Those with no documentation are considered unvaccinated and must follow all infection prevention measures for non-vaccinated caregivers as applicable.
- Respirator training - Respiratory Protection (PAPR or N95 Fit Mask Testing), if required by setting or functions performed. If prior training is not for device provided by PH&S, PH&S will provide training/testing as appropriate.
No. The Providence policy applies to vendors entering Providence facilities. Vendors who are fully remote are exempt from these requirements and are not required to provide vaccination or exemption information.
No. These vendors are exempt from tracking. Screening and tracking not required if the interaction is “fleeting” (i.e., in the facility long enough to make a delivery, pick up mail, boxes, etc.)
Vendors will be subject to all local protocols (e.g. health screening, masks, social distancing).
For interactions that are not fleeting (e.g. delivery to multiple floors or in-patient care areas) vendors are required to provide documentation of vaccination or exemption status via Vendormate or local ministry processes.
Yes. Within Vendormate there is an option to upload exemption information and related testing information.

Employers are responsible for reviewing and approving exemptions per state and/or federal guidance. Providence cannot approve these exemptions on their behalf.
Please contact your employer for more information on their process for evaluating exemptions.
If you receive an approved exemption from your employer, please upload this to Vendormate or provide it to the ministry in accordance with their local processes.
Vendors must meet all requirements noted in Vendormate which are stipulated by Providence policy and other state and local health requirements.
The requirements will include COVID vaccination/exemption documentation and other information that meets Providence policy and/or government mandates.
