Remote Patient Monitoring
Remote Patient Monitoring (RPM) is an effective method of monitoring patients with certain diagnosis seven days a week. The RPM team in collaboration with our nursing team identify potential issues before they become a problem and initiate additional same day skill nursing visits when needed.
Monitoring is done through iPad, Bluetooth, in- home scales, pulse oximeter and telephone devices. Our registered nurse-staffed call center monitors and educates patients and caregivers on vital changes in blood pressure, pulse, weight, nutrition, medication that may require immediate attention, and when to contact our team for a same day visit. Our clinical team will facilitate or initiate contact with your physician to identify issues that require medical intervention. This service prevents re-hospitalizations and allows patients to stay at home knowing that they have the oversight of a nurse on a daily basis.
