Process for Provider Board Certification Validation

• Billing logs (from a provider's own clinic) can be used to generate a case log for board certification.
• Once your Case Log is prepared and ready to be validated by HIM (Health Information Management), please e-mail your document to Mathew Liden at [email protected].
• Your Case Log will be validated and signed and then returned to you within 3-5 business days.
• If you have questions or need additional assistance, please reach out to your site-specific Medical Staff Coordinator.
