These forms and worksheets are included in the Understanding Pain resource manual. If you have been participating in the program and have used up the copies of these forms included in your manual, you can download and print fresh copies here. If you have not participated in the Understanding Pain program but think that these forms might be helpful to you, please help yourself to the forms.
- Pain Diary
This is a form which you can use to track a wide variety of factors which might impact your experience of your baseline level of pain. When you have a better understanding of the factors which influence your baseline level of pain then you will be in a position to do something about those factors. - Self Observation of Pain Triggers Exercise
This is a form which you can use to track a wide variety of factors which might impact your experience of your baseline level of pain. When you have a better understanding of the factors which influence your baseline level of pain then you will be in a position to do something about those factors. - Functional Goal Planner and Tracker
This form can help you to identify personal Functional Goals (physical activities/bodily movements which you would like to be able to engage in but have given up or reduced because of pain) and will also help you to develop step-by -step Action Plans for gradually moving toward achieving the functional goals you have identified. - Assertiveness Scripting
This form can be helpful if you are working on learning how to more effectively communicate with other people, especially in difficult situations. Remember that difficult interactions with others are stressful and that stress pushes pain levels up.
