Providence Archives maintains nursing school transcripts for:
- Sacred Heart School of Nursing, Spokane, Washington
- St. Peter Hospital, Graduate Nursing Program, Olympia, Washington
- St. Peter Hospital, Practical Nursing Program, Olympia, Washington

Ordering a transcript:
Please email Providence Archives to determine the applicable service fee before submitting your transcript request by mail.
Charges that may apply:
- Minimum fee for retrieval of one box of offsite records: $35 (plus $5 for each additional box)
- Minimum fee for official stamped transcript copy: $10 (plus $5 for each additional official copy)
Once fees have been determined, please write by mail to the Archives with the following information:
- Full name (including maiden name, if applicable)
- Dates of attendance/graduation
- Address(es) of where to send the transcript(s)
- Signature

- School attended
- Full name (maiden name, if applicable)

- Signature
A service fee of $10 is charged for each official copy requested (plus $5 for each additional official copy).

Montana
- Columbus Hospital (Great Falls, Montana); 1894-1968
Transcripts may be kept by the Montana Board of Nursing as part of the record of an active nurse. Files of deceased nurses are typically purged so likely do not exist in this office. Contact the board for more information at [email protected] or visit their website. - St. Patrick Hospital (Missoula, Montana); 1906-1978

Oregon

Records no longer exist; they were designated for destruction by the State of Oregon when they reached 75 years old. - Providence Hospital (Portland, Oregon); 1944-1962

Application files for registered nurses are available through the State of Washington Public Disclosure Department of the Department of Health. Contact the Washington State Department of Health, Nursing Care Quality Assurance Commission at [email protected] or call 360-236-4700 for general information. Records are only kept for 75 years.
