Our clinic is proud to have three providers who serve as the leading state-wide authorities in muscular dystrophy, multiple sclerosis and musculoskeletal medicine.
We understand that your pain is real, but we also believe your pain is controllable with the proper treatment and medical guidance. We are here to support you along your journey to feeling better.
We provide:
- Multiple sclerosis services - our goal is to help improve the quality of life for patients living with multiple sclerosis (MS) – and their families.
- Physiatry - our physiatrists' goal is to make patients feel better and be able to live well by restoring function that's been lost due to injury, illness or a disabling condition.
If you are diagnosed with a neuromuscular disease, your physician may refer you to Centralia Physical Medicine. You may also contact the clinic directly if your diagnosis is confirmed.
Please fill out our new patient paperwork prior to your first visit. If you need to fill it out at the clinic, please arrive at least 15 minutes early.
If advised, please fill out a patient form concerning electrodiagnostic treatment.
If you have questions regarding your insurance coverage, please contact your insurance company directly. Every plan differs in benefits. We do accept Medicare assignment. We also accept contract rates for those health plans where this has been negotiated. Co-payments are due at time of appointment.
Please update us immediate if your insurance changes. We will assist you in obtaining needed prior authorization for services if you make us aware. However, you are financially responsible for the medical rehabilitation services provided to you. In the event your health plan determines a service is “not covered”, you will be responsible for the complete charge. Payment is due upon receipt of your bill.
We reserve your appointment time for you. We require 48 hours notice if you are unable to come to your scheduled visit. We may not be able to reschedule you if such notification is not received. Please let us know if an unanticipated event comes up as soon as you know.
Sportsmetrics™ is an ACL injury prevention course proven to reduce traumatic knee injuries in athletes. This six-week course improves strength and agility for prevention.
We perform video analysis to pre-test the athlete's landing form, both jumping and landing. We then follow a six-week session of plyometric training and leg strengthening specific for their sport, the athlete will be given another video post-test to see their improvements to lessen strain to their legs and knees thus improving performance in their desired sport and decreasing the chance of serious knee injuries.
A home program is also given to continue the athlete's peak performance following the six-week course. This program will improve strength, flexibility, quickness and your vertical jumping all while reducing possible injury.
Your video pre- and post-tests will last 45 minutes. Each training session is between 30 and 45 minutes, three times a week for six weeks.
