Community Connections
Community Connections is a ministry of Providence Seaside Hospital that transports people to services and appointments related to your health and well-being.
Originally called Partners for Seniors, Community Connections now serves people of all ages who have no other transportation options. The program helps you maintain independence while getting you to health care and wellness activities.
Community Connections offers transportation to:
- Medical appointments (locally and in the Portland area)
- Grocery stores
- Post offices
- Gyms and pools
- Banks
- Libraries
- Other locations as needed
When scheduling a ride, people who need transportation to a medical appointment get priority. We ask for seven days’ notice to schedule a ride. All rides are subject to the availability of drivers. All services are offered free of charge.
We serve people in our community who have limited options for transportation. We ask you to reach out to your support network first before seeking our services.
People who receive Medicaid transportation funding are not eligible to use our services to medical appointments because transportation is already provided by another agency. If you need assistance, we can help you sign up for eligible transportation.
Providence Community Connections is funded by Providence Seaside Hospital, but it relies on the gift of time from volunteers in our community. Most volunteer tasks require two to three hours to complete. The schedule is flexible and volunteers decide how much time they can offer.
