Providence Oregon Hospice
Our Approach
People facing the end of life often need special care, and their families and loved ones may need additional support as well. Common end-of-life desires include avoiding suffering, last wishes followed and dying with dignity.
Providence Hospice of Oregon provides expert, compassionate care for individuals as they face these stages in their lives. We understand that each family is special and tailor our team approach to their specific needs.
Our hospice experts work with your primary caregivers to provide patients with the support needed to spend their time living as fully and completely as possible, in your own familiar surroundings, and in the company of family and friends.

Your hospice team brings much of your medical care to you at home—they become the eyes and ears of your physician in your home, with the added benefit of hospice expertise. Your hospice care includes access to experienced nurses available by phone 24 hours/day. Hospice is covered by Medicare, Medicaid, and most private insurance. Learn more about Oregon Hospice.
Hospice is not 24/7 caregiving for your day-to-day needs, although your team can help you find this kind of help. You can disenroll from hospice at any time—it’s your choice. Disenrolling from hospice does not make you ineligible to re-enroll later. You do not have to be DNR to enroll in hospice. Learn more to determine the level of care you or your loved one needs.
Anyone can request hospice services. This includes a family member, friend, physician, neighbor, or the person themselves. We will contact the provider and determine if the person qualifies for hospice services. A hospice nurse will then call to discuss our services and answer your questions. The nurse can also arrange for an informational/admission visit at your place of residence.
To prepare for the informational/admission visit, please have handy:
- A list of all medications you are currently taking, including over-the-counter medications and vitamin and herbal supplements.
- A copy of your POLST and advanced directive, if you have signed one.
Referral forms and information
Routine care is the hospice service provided by your Hospice Team in the comfort of your place of residence, whether it is in a facility or your own home. The services are intermittent and interdisciplinary and are based on your care plan.
Respite care is available upon advance notice and by request, to provide your family and caregivers with a break. It is provided in facilities contracted with Providence Hospice.
Continuous care and Inpatient care are available as part of your Medicare Hospice benefit when an urgent situation develops and your family or current caregivers are unable to provide the needed medical care to reach comfort. Custodial care needs do not meet the eligibility for these levels of care.
On-call nurses and other interdisciplinary hospice team members provide you with access to hospice staff twenty-four hours a day, seven days a week, including holidays.
Providence Hospice offers several services to help adults, children and families through the process of grieving. These services include the following:
- Grief classes
- Hospice Bereavement Services offers community care seminars. These are free workshops available to groups, explaining practical skills for supporting a friend or loved one facing a serious illness or loss
- Grief support groups
- Camp Erin: A unique camp designed for children who are grieving
Volunteers are essential to the special care Providence Hospice provides. Whether a volunteer is helping families, visiting patients, consoling the bereaved or assisting with office tasks, each contributes greatly to the quality of life for the people we serve. We ask that volunteers make these commitments:
- To contribute one to four hours a week, depending on the area of service
- To commit to a minimum of one year of volunteer service, if possible
