Population Health Management
Population Health Management (PHM) at Providence uses aggregate population-level data to develop evidence-based interventions resulting in better patient outcomes, elimination of health inequities, and increased economic sustainability. Through this data-informed approach, we prioritize and proactively manage groups of patients to improve health outcomes, drive effective use of resources, partner with our communities and optimize performance under new models of care and payment models.
We support stakeholders across the system in order to do well in value-based care (VBC) arrangements. We serve as advocates for Providence by ensuring all internal teams and payers have the data, performance information, and actuarial insights to operationalize and succeed in these arrangements. We provide a portfolio perspective on the scale, risk, and performance in VBC.
We do bi-directional data exchange and enrichment with payers so that caregivers can have the information needed to manage patient populations.
We work to optimize Providence's performance in all value-based CMS programs including MSSP, MIPS, Primary Care First and others. We do this by helping providers determine which program is the best fit, help them to understand the parameters of the program, identify opportunities, assure program compliance, and accurate data exchange with CMS.
We support strategy and performance management including the development and execution of the department strategic plan and establishing relationships with key stakeholders throughout the organization for optimal alignment with the strategic priorities of Providence. We provide program and project management support for various initiatives within the Population Health Management department to support the expansion of value-based care.
